Practice Education Series

The Wisconsin Medical Home Initiative offers education to physicians, their care teams, and families on medical home topics. Outreach is guided by policy statements or clinical reports from the American Academy of Pediatrics (AAP). These no-cost, one-hour trainings are delivered on-site at primary care clinics, at a time that is convenient for clinicians and care team members.
