The Wisconsin Department of Health Services' Title V Children and Youth with Special Health Care Needs Program is sponsoring a Care Integration: Models, Tools, and Measurement statewide summit supporting medical homes and youth health transitions. This summit is offered for health care and community resource professionals who work with children, youth, and young adults with special health care needs and their families. Keynote speaker will be Richard Antonelli, MD, MS, Medical Director of Integrated Care and Physician Relations and Outreach at Boston Children's Hospital.
